If you haven't updated your drivers, do so, if you have, it could've gone wrong.
If neither of those, slowly RIP graphics card.

Exact same thing happened to my friends laptop when one of his RAM cards somehow got damage. He removed it and it stopped happening. But i think what quotary said about a damaged gpu could be true as well

I updated all and turned off / on (restarted) my laptop and still the same :'( 

P.S. i only have Intel HD Graphics :'(

I can't removed RAM card in my laptop because it is tampered in motherboard :'( meaning the RAM cannot be removed and replaced...
