Also make sure your power supply can handle it as a 400w generic thing will shit itself in no time.
Possibly considering replacing the psu too a 500~600w brand name one

GTX 1060 or RX480

Getting one of those will be more than enough for most games

If you want, do what I did. Spent $60 on Radeon HD5450, running GTA V on minimal w/ roads disappearing sometime. Playable, 10/10.

lolz a 5450 isnt even worth $10 imho

All suggestions so far are good and seem to be around the $200ish USD mark which is what you should aim for these days to spend on a video card at the minimum.

you dont need a top of the line, latest graphics card for $150 when one 1 or 2 generations behind for $100 less
would work perfectly fine... Im still with shane here that if you upgrade your GPU youre going to need to upgrade your PSU... dont end up like me and have a stock 450 watt PSU and buy a 4GB video card just for the thing to burn out (PSU) and have it cost you $75 additional for a new one (replacement GPU)

Just my opinion, i would go for a 250GB SSD, i have a 120 and it bugs me now that its full and i have to install my programs and games on the hard drive.
